中肾样腺癌（MLA）是一种主要发生于子宫或卵巢的罕见肿瘤，恶性程度较高。本文报道了一例卵巢MLA伴交界性浆黏液性肿瘤的影像学表现及诊疗经过。患者2年前行超声发现右侧卵巢囊性肿物，后因下腹部膨隆1个月余行超声、CT和MRI影像学检查，显示右侧卵巢肿物的实性成分较前明显增大，卵巢-附件影像报告和数据系统（O-RADS）MRI评分为5分。患者经过手术和化疗，目前随访2年无复发和转移。盆腔影像学尤其是O-RADS风险分层有助于MLA的术前评估。
